2018-19 COI Disclosure for PHS or NIH projects: fill out at time of applying for funds

This COI disclosure is for research funded by Public Health Service (PHS) agencies (this includes NIH, FDA, USDA, CDC, NIFA, IHS, etc.) which is performed at UNM Main Campus or Branch Campus/es. You must fill out this disclosure each time you submit a proposal for funding to PHS or their affiliate(s). If you submit several PHS funding proposals, you are required to file this COI disclosure at the time of each proposal submission. PHS disclosures are project-specific and separate from your annual COI disclosure.

Each disclosure will receive an initial review and you will be notified of the determination after the review is completed. If the disclosure requires a review by the full COI Committee, you could be asked to be present at the Committee meeting.

If in the course of submitting your disclosure you need to stop and complete your disclosure at a later time, please follow these instructions. Failure to follow them will result in loss of information entered. Select the command, 'Save & Complete Later'. Enter your email address in the field provided and click 'Place Survey on Hold'. The survey will be placed in a hold status and you will be sent an email with a subject line of 'Survey Return Ticket'. The email will have a link to the survey you started. You will need that link to again access your partially completed survey.
