Profile form is the page where panelists enter information about themselves. The form is available to panelists as a part of the "Edit profile" screen in the panelist portal.

The Profile form as created when the panel design is complete. Click on the "Create profile form" button on the bottom of the "Edit attribute" or "Edit categories" screen. You can preview Profile form by clicking on the preview link in the Menu box from panel screens.
